How to make a QR code for your office for customer health declaration during the Covid-19 pandemic


A QR code can raise the possibility of customers checking in before entering your office, therefore easy for you and the Government to keep track of the number of people checking in/ out and contact those exposed to Covid-19.

You can make a QR code for your office within the five following easy steps.


1st Step: Access the website for the health declaration of Vietnam Ministry of Health => Click on "Đăng ký điểm kiểm soát QR Code"


2nd Step: Enter your phone number to receive the OTP code => Click on “Nhận mã OTP


3rd Step: The OTP code shall be sent to you by phone message, then enter the OTP code => Click on Tiếp tục


4th Step: Enter all information required


5th Step: Enter the secret code as shown in the form => Click on “Đăng ký” to finish.

Notification after completion shall present as the photo. Click on the icon to download/ print the QR code.
